For Students Who Need Health Care Plans (HCP) and/or Medications at School or School-Sponsored Events:

All students who plan on taking medications at school need a Health Care Plan (HCP). You will find links below to HCP forms for the most common medical conditions. If you do not see a plan for your student’s medical condition, please contact your regional nurse. Colorado state law governs our medication policy. Medications include prescription, over the counter, and homeopathic medications need a Health Care Plan to be administered.
PLEASE NOTE: IF THE SCHOOL DOES NOT HAVE THE REQUIRED HEALTH FORMS FOR YOUR CHILD’S MEDICAL DIAGNOSIS, WE ARE NOT ABLE TO GIVE YOUR STUDENT MEDICATION AT SCHOOL AND WILL NEED TO CALL 911 IF SCHOOL STAFF DEEM IT AN EMERGENT SITUATION.

Allergies

GOAL High School does not restrict students or staff from bringing food to school. We encourage students and staff not to share food, to wash hands before and after eating, and to check food labels prior to eating. Students who have severe allergies and need medication at school may provide their regional nurse with a completed Severe Allergy and Anaphylaxis HCP.

Immunizations

Colorado law requires all students attending Colorado schools to be vaccinated against certain diseases unless a medical or non-medical exemption is filed.  Parents/Guardians/Adult students are responsible for providing complete immunization records or an immunization exemption.

Non-medical exemptions MUST be completed at the start of each school year. They expire on June 30th of every year.

Please click on the Colorado Department of Public Health Environment link below for instructions on how to obtain an immunization exemption.

Concussion Care at Goal High School

Each year in our country, thousands of 9-12 students sustain a concussion from a fall, motor vehicle accident, sports field or other activity. Goal recognizes the growing concern with injuries resulting in concussion or concussion-like symptoms.

If your student experiences a concussion or you are concerned about a head injury, please contact your school nurse to review.

Goal High School is fully committed to the academic success of all our students, and support for all health conditions that may impact learning. Goal uses evidence-based guidelines to help in the support with students who are impacted by a concussion, however you and your student’s communication and support are integral to assist us in offering the best possible outcome and support for your student in school.

What this means for all Goal students:

  • Education to staff regarding concussion identification and support.
  • Removal of the student from school day physical activities (beyond walking).
  • Initiation of Teacher Acute Concussion Tool (TACT) to each teacher who has a student in the class.
  • Return to activity (play) with the medical provider’s direction or a completed parents approval letter to clear the concussion care at school.

Your student’s school nurse will work closely with your student and you to help monitor healing and assist staff with updated information on the status of your student’s concussion.

A weekly symptom self-screener will need to be completed by your student.

A healthcare provider clearance note, or a parent approval letter will be needed to fully complete concussion care at school.

If your student still is experiencing symptoms or concussion related challenges in school after 4-6 weeks, further support is available and will be utilized.

Some online suggestions for parents and students from Colorado Department of Education include:

  • Create a routine and encourage sticking to a daily schedule
  • Discourage student naps – it is important to maintain a normal sleep schedule
  • Consider using blue-screen glasses while online.
  • Use word prediction software that predicts words while the student is typing such as:
    Co:Writer
    TypeIt4Me (Macs only)
  • Reduce visual clutter on the screen pages to reduce visual overload
    Immersive Reader
Safari Reader on the Safari browser
Mercury Reader extension on Chrome browser
  • Text-to-speech for a student who may find reading too taxing they can have books or documents read aloud. Examples include:
    Snap and Read
    Audio Books: Audible, Scribd
Reads other types of documents- NaturalReader, Voice Dream Reader
  • Speech-to-text – Speech recognition software that will turn a student’s speech into text on the computer screen.
Siri’s dictation feature offers real-time speech-to-text conversion, recording the words you say as typed text on Apple products.
Mac Speech to Text (Apple products)
    Dragon
